Job Description:

We are seeking a skilled Patient Care Manager to join our team at Mederi Caretenders Home Health. This role is ideal for individuals who share our passion for delivering high-quality care to patients in their homes. As a Patient Care Manager, you will be responsible for providing clinical services, supervising staff, and ensuring patient needs are met. Key Responsibilities:

Supervise and coordinate clinical services to ensure high-quality patient care.
Collaborate with healthcare teams to develop and implement patient care plans.
Conduct regular assessments to ensure patient needs are met and care is individualized.
Ensure compliance with laws, regulations, and agency policies.Requirements:
- Current RN licensure in state of practice.
- Current CPR certification required.
- Current Driver's License and access to a dependable vehicle or public transportation.
