Job Description:

We are seeking a skilled Patient Care Manager to join our team at Mederi Caretenders Home Health in Brooksville. As a member of our interdisciplinary team, you will be responsible for the overall supervision and coordination of clinical services.Key Responsibilities:

• Coordinate and supervise an interdisciplinary team of staff to ensure the continuity of high-quality care to home health patients.
• Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
• Develop, implement, and update individualized patient plans of care as necessary.
• Manage and document phone calls from physicians, clinicians, patients, referral sources, and communicate patient updates/new orders to clinicians.Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
