Youll work under the direction of a physician and nursing supervisor, ensuring care meets all regulatory and clinical practice standards.*Key Responsibilities** Perform initial and ongoing patient assessments, including OASIS documentation.* Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.* Collaborate with the interdisciplinary care team to optimize patient outcomes.* Provide education and support to patients and families on therapy programs.* Supervise Physical Therapist Assistants every 30 days.* Communicate patient progress and changes with physicians and care teams.* Document all visits and progress notes on the same day of service.*Qualifications** Graduation from an accredited Physical Therapy program.* Active PT license in Florida.* Current BLS certification.* Valid drivers license and reliable, insured vehicle.* At least one year of PT experience (home health experience preferred).* Proficiency with electronic medical records is a plus.* Physical capability to perform required tasks (standing, bending, lifting).* Strong self-management, communication, and judgment skills.*Ready to Join Our Team?*Wed love to hear from you!Contact *Amanda Henley at AHENLEY@PinnacleHomeCare.net *for more details._Pinnacle Home Care is proud to be an equal-opportunity employer.
