Job Description:
A company is looking for a Care Coordinator - LPN to manage
telephonic outreach to chronically ill patients and develop
personalized care plans.
Key Responsibilities
Manage patients' healthcare needs via virtual phone
conversations
Document visits using technology platforms and EHRs
Coach patients on treatment plans and connect them with
resources
Required Qualifications
Active Compact LPN License, or CA or IL Single State LPN
License
Minimum two years of clinical experience; care coordination
experience preferred
Proficient with Electronic Medical Records and Microsoft Office
